Recoverable solve outcome; counters include attempted operator calls.

A residual is present only after successful evaluation of an iterate. Later numerical failures leave that iterate and diagnostic intact. Iterations count attempted Arnoldi steps; cycles count started restart cycles, excluding the initial residual check.
